assistance to engage leaders across CCOs, public health, and community-based organizations involved in health system transformation. The discussions will include local and national experts who will share content related to models of care for heart disease,
diabetes and prediabetes.<o:p></o:p></span></p>

<p class="MsoNormal" style="mso-margin-top-alt:auto;mso-margin-bottom-alt:auto"><span style="font-family:"Calibri","sans-serif";color:#212121">The SRCH Institutes will support community partnerships in developing formal commitments, such as memoranda of understanding
and data-sharing agreements, to reinforce collaboration and a long-term commitment to health system improvement and community-clinical linkages.<o:p></o:p></span></p>
