*About this Webinar:*
This webinar will feature an over of the impact of Remote Monitoring and
Chronic Care Management for Community Health Centers and their patients,
along with stakeholder collaboration on the use of telehealth within an
integrated care model. Roanoke Chowan Community Health Center implemented
the first remote monitoring and chronic care management program within a
Federally Qualified Health Center for Cardiovascular Disease, Diabetes and
Hypertension Patients.



*Presentation by: Kim A. Schwartz*
Kim A. Schwartz has served as the Chief Executive Officer of Roanoke Chowan
Community Health Center since June 2005. Kim received her MA degree in
Counseling Education, Multicultural Honors, from the University of New
Mexico in 1996. Prior to her current role with RCCHC, Kim served as CEO for
the Guidance Center of Lea County in Hobbs, New Mexico (a comprehensive
community mental health center that expanded into primary care as an FQHC
Look-Alike under Kim's leadership). In the course of her health services
career, Kim has served in many leadership positions. Kim has practiced as a
licensed psycho-therapist and her professional passions include the
integration of behavioral health and primary care and telehealth remote
patient monitoring. Kim has served on various boards/councils, which
include Hertford County North Carolina Social Services Board, Hertford
County Emergency Preparedness Council, North Carolina Care Share Health
Alliance Board, North Carolina Community Health Center Association Board
Treasurer, New Mexico Governor's NM Behavioral Health Advisory Council, New
Mexico Psychologist Licensing Board and New Mexico Behavioral Health
Steering Committee Ad Hoc. Kim has many affiliations and frequently
presents at national and regional conferences which include, American
Telehealth Association, Mental Health and Remote Monitoring Special
Interest Group Member, Bureau of Primary Health Care Objective Review
Board, Bureau of Primary Health Care New Start Executive Leadership
Learning Team, NC ICare Partnership Advisory Committee, National Rural
Health Association, National Association of Community Health Centers,
National Council of Community Behavioral Healthcare, National Association
for Rural Mental Health, and National Board of Certified Counselors.
