Utilizing Telehealth for Patients with Chronic Heart Failure

Expanding access to its successful telemonitoring program helped the VNA of
Rockford, Ill. lower readmission rates to as low as 4% for high-risk
patients being treated for chronic heart failure at Rockford Memorial
Hospital.

VNA staff and hospital cardiologists developed a new Heart & Vascular
Program <http://info.hommed.com/Y0o0j0V47000T002LBS0bJ0>, which established
monitoring protocols & standards for all patients treated for CHF,
including those who did not qualify for telemonitoring under Medicare
guidelines.

*  Success Factors*:

   - A transition coach to assist patients
    - Patient interviews by cardiologists
   - Customized monitoring parameters to enable a fast & targeted response
   to each patient's symptoms.
    - LifeStream remote patient monitoring software to easily implement
   unique patient monitoring plans for all CHF patients.
    - Long-term monitoring to encourage healthy behaviors and patient
   engagement.

    Since the program began, CHF readmission rates have been reduced by
half, demonstrating highly successful outcomes for participants.
