New Models of Care- Looking at PCMH & telehealth

   - Brief background-  IOM- Crossing the Quality Chasm, Institute for
   Health Care Improvement work, escalating costs, increasing errors, mediocre
   quality.  Then ACA, new models of care.
   - PCMH care model over 20 years old, successful in pediatric setting.
   Seen today as one of most promising care model.
   -  Will show care the “Old Way” and the “PCMH way”.
   - Explain a few models at CHCs in the Montana
   - Telehealth implications

 *Presentation by: Paula Block, RN, BSN*
Paula Block, RN, BSN, is a Clinical Process Improvement Manager at the
Montana Primary Care Association, MPCA.  MPCA is an association of 18
Community Health Centers or FQHCs, Federally Qualified Health Centers.
Paula provides training, technical assistance and coaching to primary care
practices in quality improvement, risk management and PCMH
transformation.   Paula can help a clinic assess, strategize and implement
change to improve clinic systems.  She has experience helping centers
achieve PCMH recognition, Joint Commission Accreditation, and meeting HRSA
Community Health Center requirements and expectations.   She leads peer
networks of medical directors, dental directors, clinic quality leaders,
behavioral health integration staff, and clinical pharmacists. She is a
registered nurse.  She can be contacted at pblock at mtpca.org.
