[TAO] New fact sheet for CMS policy during COVID-19

The Center for Connected Health Policy has done a great job of putting
together a single fact sheet about current federal telehealth policy during
COVID-19.  Please note that there is also some information about telehealth
policy changes being implemented at the state level.  I will send the
Oregon changes to CCHP to see if we can have them added.
