During an open forum September 10, 2013, the topic was discussed that SPOTS Cardholders' Social Security Numbers (SSN) are on file with US Bank. The request to remove those numbers has been agreed upon. DAS EGS FBS SFMS would like to have the SSN information removed from the US Bank website and replace it with the employee's OR numbers. An automated system has been put in place in order to accomplish this process. DAS EGS FBS SFMS has included a link to the updated SPOTS application form that is now available on the OAM website: SPOTS Card Application and Agreement and the SFMS website: SPOTS Forms. SFMS would like to implement and complete this project the week of December 16, 2013. DAS EGS FBS SFMS has the following question: Does this give agencies enough time to notify cardholders that they will need to provide their OR number instead of their SSN when contacting US Bank after this date?
