Registration for the Oregon Self-Management Forum on May 8, 2013 at
the Airport Sheraton in Portland is now available. This conference,
formerly known as the Living Well Forum, is expanded in scope this
year to include a wide variety of evidence-based chronic disease
self-manageent programs and resources. The agenda is geared to appeal
to a broad audience of those involved with self-management program
provision, coordination, promotion and funding in Oregon. Attendees
will include self-management program leaders, master trainers and
coordinators; public health and social services professionals; health
system representatives (including clinical providers and staff),
insurers, employers, and other interested community partners.

An important note: There will be a $50 registration fee for the forum
this year.  To assist those with need, there will be limited
scholarship opportunities available, which can be requested through
the online registration process.

Forum participants traveling 70 miles or more to Portland are eligible
for a paid hotel room the night of May 7, 2013.  You can make your
hotel request in the Forum registration process. A credit card number
is required to hold your reservation, but the card will only be
charged for no-shows who do not cancel their hotel reservation with
sufficient notice.
