[Libs-Or] ID required for patron registration

I'm surveying Oregon Libraries as to what they require for identification when registering a patron for a Library card. For our Library, there are two criteria that the identification must address- the first is a picture ID that proves they are who they say they are, and the second is some sort of proof that they live at the address that they give us (utility bill, first class mail addressed to them in the last 30 days, etc.) If they have an ODL or Oregon ID with their current address, we are happy - but often, that is not the case.

What do you require? Do you take the temporary paper driver's license? For photo ID, do you take a Costco card? An expired ODL? ......etc?  I'd appreciate your input.
