[Libs-Or] Tracking in library magazine use

We are working to track our in library magazine use this year. We have been looking at the idea of affixing a note to each magazine along the lines of:
'Please add an X to this sheet if you have used this magazine.'

Do you have something you have used, at your library, and found successful? We are looking for input.
