[Libs-Or] ID requirements for library card

We require photo ID and proof of a street address in our service area for a full-privilege card. In the absence of this, folks can get a provisional card that allows for two checkouts at a time; after four months, they can request to have their card upgraded to a full-privilege card. The reasoning there is that we figure by that point, they're clearly residing in the area even if they don't have a permanent address.

We are curious as to what other libraries require as far as ID to get a library card.  Do you require proof of address?  If the potential patron is using a post office box as mailing address, do you also require a physical address?  If so, do you require proof of physical address, or just proof of mailing address?
