[MyOEBB] Establishing HSA accounts with U.S. Bank

Dear Superintendents, Business Managers and Benefits Administrators:

 

We've been getting a number of requests for information on how to
establish accounts for employees who choose to participate in the HSA
accounts available through U.S. Bank beginning October 1, 2011.  

 

Starting this process as early as possible will help U.S. Bank establish
accounts for all employees who desire one by the October 1, 2011,
effective date.

 

*	To learn more about establishing these accounts for your
employees, please go to: www.oregon.gov/oha/oebb/entity/HSAsetup.shtml.

 

*	If your entity has confirmed your EE Plan Management selections,
but did not select ODS Medical Plan 9 and would like to add it, please
contact Linda Tullis by phone at (503) 378-3329, or by e-mail at
linda.tullis at state.or.us as soon as possible.  If your entity did not
select ODS Medical Plan 9 and did not intend to do so, you can disregard
this message.

 

You are also invited to attend one of the upcoming phone conferences
hosted by U.S. Bank to assist OEBB educational entities with this
process:

                

*	Tuesday, June 28, 2011 (1pm - 2pm PST)

*	Tuesday, July 5, 2011 (1pm - 2pm PST)
*	Tuesday, July 12, 2011 (1pm - 2pm PST)

 

No registration necessary. To participate, just call in during one of
the above times. Dial 1-877-747-4538, then when prompted, enter the
passcode: 3481509.

 

In these phone conferences U.S. Bank will give a general overview of the
benefit and walk you through the forms, answer any questions you may
have and give you a preview of what to expect down the road.
