[Announcements from OSPS to the users of the Oregon State Payroll Application]

NEW STOP PAY FORM
We discussed the new stop pay procedure and form at our user forum on
Wednesday.  Beginning Monday, March 2nd, you will see a new form on our
Web site.  The new stop pay form replaces the need for an employee
signature to replace a lost check over $50.00.

You can complete and scan the form to send to us via e-mail, or print
and fax it to us.
