Subject:           New EEOC 4 Self-Identification Form

We have posted the updated "Employee Voluntary Questionnaire for Self-Identification of: Race/Ethnicity, Disability and Veteran Status" (attached). The form is located on the HRSD website under Forms at: http://www.oregon.gov/DAS/HR/pages/forms.aspx

This form is used to data collect data from new employees. The data is subsequently entered into the Personnel Data Base.
The data is collected as required by the Equal Employment Opportunity Commission and for other statistical and reporting purposes.

Please forward this email to those HR or Payroll staff who are responsible for collecting the data from employees
