[Self-Management_OR] Support for the Diabetes Self-Management Program!

As part of the Medicare-sponsored Everyone with Diabetes Counts (EDC) Initiative, Acumentra Health is seeking communities who are interested in receiving support with

·         Establishing or growing the Diabetes Self-Management Program

·         Increasing provider referral to self-management education

·         Improving data collection

·         Connecting with provider education opportunities related to diabetes and cardiac care

The EDC initiative aligns with priorities outlined in the Oregon Diabetes Report: A report on the burden of diabetes in Oregon and progress on the 2009 Strategic Plan to Slow the Rate of Diabetes (January 2015)​<https://public.health.oregon.gov/DiseasesConditions/ChronicDisease/Diabetes/Documents/OregonDiabetesReport.pdf>.

As detailed in the latest Q-News<http://www.acumentra.org/q-news/everyone-with-diabetes-counts-2/>, the EDC initiative supports state priorities by

·         Increasing the availability of diabetes self-management education in Oregon. EDC focuses on improving patient access to the Stanford University Diabetes Self-Management Program<http://patienteducation.stanford.edu/programs/diabeteseng.html> (DSMP). Acumentra Health is also working with local providers to establish or improve clinical referral systems to the DSMP.

·         Improving the quality and standardization of data collection and analysis. Acumentra Health is working with communities to

o   help providers document and report key clinical measures of diabetes and cardiac care (diabetes is a leading cause of heart disease and stroke)

o   standardize data collection on patients’ understanding of how to manage their diabetes before and after completing the DSMP
