Hello Everyone,

NorthWest Senior and Disability Services, Acumentra Health, and Columbia County Public Health Foundation are putting on a series of Leader Trainings in St. Helens. Please find attached a flyer for two Stanford evidence-based Leader Trainings.  These classes are free to leaders and are being offered over a 3-week period.  Here are the details

Training #1: Stanford's Chronic Disease Self-Management Program (CDSMP) LEADER TRAINING
Dates:  May 14-15 & May 28-29, 2015  (Thursdays & Fridays)
Time:  10am-5pm for all 4 days
Place:    Columbia County Soil & Water Conservation District Meeting Room

Training#2: Stanford's Diabetes Self-Management Program (DSMP) LEADER CROSS-TRAINING
Dates:  June 4-5, 2015  (Thursday & Friday)
Time:     9:30am-4:30pm for day 1 and 9:00am - 12:00pm for day 2
Place:    Columbia County Soil & Water Conservation District Meeting Room
