Our friends across the river in Washington asked me to send this out - there are some spaces potentially available for folks from Oregon. I've attached the flyer but the application form is too large as an attachment, so please contact Lexie for that. Her information is listed below.



Leader Training:

Chronic Disease Self-Management Program

(AKA: Living Well with Chronic Conditions)

PeaceHealth Southwest Medical Center (PHSW) and Area Agency on Aging and Disabilities of Southwest Washington (AAADSW) are pleased to offer leader training for Stanford's Chronic Disease Self-Management Program (CDSMP or Living Well).



When: February 11, 12, 18, 19, 2015 9 am - 5 pm

Where: Trinity Lutheran Church, 309 W 39th Street, Vancouver WA 98660

Cost: Free* (including continental breakfast and lunch each day)

Master Trainers: Marilyn Johnson and Kelly Pick from PHSW



Prospective leaders must:

* Attend a Living Well workshop as a participant if one available prior to leader training (may attend at PHSW or check Washington Living Well or Oregon Living Well for other options)

* Submit completed Application no later than January 21, 2015

* Have interview no later than January 28, 2015

* Have satisfactory Washington State Patrol criminal background check

* Attend entire training (all four full days)

* Commit to leading first workshop within 6 months of completing leader training (workshop to end by August 31, 2015)

* Plan on leading at least one workshop per year
