[Self-Management_OR] Living Well Credit for Adult Foster Home Providers

Hello!

Thanks to initial work done by Deschutes County Health Department, I was able to reinstate the statewide approval for Adult Foster Home providers to receive 2.5 credits for attending Living Well workshops.  These credit approval procedures need to be completed annually.

Providers from aging/physical disabilities, addictions and mental health, or developmental disabilities foster homes are all eligible for this credit, and will just need to ensure that leaders sign off on paperwork they'll provide that indicates they've attended.  These staff provide critical care to others, and it is wonderful that they can receive credit for learning skills to self-manage their own chronic conditions and/or help the ones they support with their on-going health issues!
