 Dear Oregon Stakeholder,

The first Oregon Medicaid Electronic Health Record (EHR) Incentive Program's
stakeholder survey is now available.  Your assistance is greatly
appreciated.

This web-based survey is intended for providers, clinics, hospitals,
provider associations, Medicaid health plans, and other stakeholders to
provide input as Oregon's Medicaid EHR Incentive Program is developed.

The results of this survey will be compiled into a summary document to be
distributed to the Medicaid Health Information Technology (HIT) leadership
team and other decision makers, and will be available to the public on
