[yscon] Survey Says...

Hi Tessa and all,

My State SRP Report addressed many of these aspects.  I am attaching a Word
doc of the questions I asked the YSC last year....but I sent them a link to
a Google Form where they actually completed the report.  I wanted them,
however, to see the questions ahead of time so they could prepare before
beginning the Form, hopefully expediting the process for them.
