[yscon] Summer Statistics Survey Question

I try to make it shorter each year - not really, but over time I've eliminated several questions for which I don't really use the data and neither does anyone else.

This year, I stopped asking for registration and completion numbers, because an increasing number of libraries no longer use traditional registration or completion methods. It's been apple-orange salad to try to make a statement about SRP completion rates statewide. Instead, I'm just asking for the number of individuals (by age group) who participated in SLP/SRP (by the library's definition of participation), and the head count at SLP/SRP-related programs.

Also, I'm no longer asking if libraries used the CSLP theme. I continue to solicit their ratings and comments on CSLP services and products ("Did not use" is a choice in the ratings question).

I know that Connecticut shared their summer reading stats survey a while ago (thanks Linda!), and I'm wondering if anyone else has one they'd share with the group? I'd love to edit our very unwieldy 28 question survey into something much more manageable and meaningful for our libraries. Any examples of info you ask for would be helpful.
