Hi Greta,

I've attached a sample of my survey as well as the text I send to our youth services staff and director's listserv; it is all cross-posted to our agency's summer reading support website:

When I began 3 years ago, the survey was around 60 questions. I've worked hard to streamline it and have goals of eventually turning it into a report and toolkit that can be downloaded and used for advocacy efforts at the local level.

Hello summer reading friends,

I know that Connecticut shared their summer reading stats survey a while ago (thanks Linda!), and I'm wondering if anyone else has one they'd share with the group? I'd love to edit our very unwieldy 28 question survey into something much more manageable and meaningful for our libraries. Any examples of info you ask for would be helpful.
