FYI-
This article in last Sunday's San Francisco Chronicle points to PG&E's inadequate GIS as one problem that led to the San Bruno explosion that killed 8 people. Here are some quotes from the article:

"Experts interviewed by The Chronicle said the problems weren't with the software but with data entry and the implementation of the ESRI programs by PG&E and its contractors."

"Reiydelle said it didn't take long on the job for him to find out that the upgrade project and the entire GIS<http://topics.sfgate.com/topics/Geographic_information_system> system had been a mess for a long time. He said only 70 percent of the data in the GIS database are accurate and that overall, it is "a really dangerously bad project." "
