Dear Partners - I'm forwarding this important update on the Hub process:

For the past few years the Oregon Public Health Division's Health Promotion and Chronic Disease Prevention (HPCDP) section has been leading an effort to establish greater sustainability for chronic disease self-management programs in the state. In 2012, we collaborated with many stakeholders to develop the Living Well Business Plan, which proposed a business model called the Self-Management Hub. This entity would develop a self-management delivery network, market programs to payers and participants, and facilitate centralized systems for payment, processing, and program scheduling and quality assurance.

Most recently, HPCDP has worked with Oregon Self-Management Network partners to develop a public solicitation for proposals (RFP) from organizations interested in developing the Self-Management Hub. This RFP is now on hold as we re-evaluate the Hub concept and make sure the proposal is optimally aligned with Oregon's rapidly changing health system transformation environment, the importance of community centered delivery systems, and coordination with Coordinated Care Organizations.

We will be back in touch with an update when the sustainability project plan has been revised. If you have questions, please communicate with your liaison. Thank you for your continued support for self-management program sustainability.
