[Announcements from OSPS to the users of the Oregon State Payroll Application]

We've published a new form for agency use in requesting W-2 corrections.
The new version can be completed online and then printed for signatures
and to submit to OSPS.  The form provides improvements such as drop-down
boxes and Excel entry fields for the correction detail.  

 

We hope you will find this new form useful and user friendly.
